STUDIO WAIVER FORM The Pink Art Pixie, LLC adores teaching all ages about art! This is a process-based art class that teaches using sensory-based experiences, art, sewing by hand, clay, fiber arts and other mixed media art project. As the parent or legal guardian of your child and by your signature below, you agree to the following: 1. Our goal is to foster a positive learning environment filled with creativity and camaraderie for all of our students. You and your child agree to treat all participants of The Pink Art Pixie with kindness and respect. 2. We love to have fun and get messy! You will make sure your child is dressed properly which includes wearing appropriate shoes and either a smock or clothing that is meant to get dirty. 3. As a process-based art studio, we use a wide range of materials in our art projects, including, but not limited to, paint, glue, pastel, chalk, fiber, wax, glue guns, hammers, and other sharp objects. While we do our best to monitor every child's use of all materials, accidents may occur due to the inherent risk in using these materials. You understand that these materials are not meant for consumption and will instruct your child not to consume any materials and to always conduct themselves in a safe manner. 4. Your safety and the safety of others is our TOP priority. You acknowledge that you have no physical, emotional, or social conditions that make it dangerous to safely participate in our programs. Running, jumping, or horseplay is extremely dangerous in the art environment and you agree to safely and respectfully move throughout the space where a Pink Art Pixie program is being held. 5. You give your permission for your child to willingly participate in The Pink Art Pixie programs. You release and hold The Pink Art Pixie harmless from any and all liability that may arise in connection with The Pink Art Pixie programs. 6. By providing your email address below, you agree to be added to The Pink Art Pixie newsletter 7. This Liability Waiver is governed by the laws of the state of Maryland.

Photo Release: We love promoting process-based art and sharing it with others. We may take photographs, video and/or audio (the “Media”) during our art classes to post on our social media. We are the sole owner of any Media and may use the Media in any way, in our sole discretion. You grant us permission to use your child’s image without any restriction and waive your right to inspect or approve the Media.
